STUDENT SUCCESS TEAM REFERRAL
TO BE FILLED OUT BY PERSON REFERRING
If your child is struggling in school and you need help or if you suspect your child might have a disability you begin with a referral to the Student Success Team (SST) of your child’s school.
The SST Team
The SST (student success team) is made up of various members of school and district staff. This can include an SST facilitator, an administrator, the school counselor/504 coordinator, nurse, mental health counselor, the special education teacher, Title 1 staff, teachers, and the school psychologists.
Students are referred to SST by parents, doctors, teachers, an administrator, or anyone on the SST team. When a person makes a referral, s/he fills out the form and turns it into the SST facilitator for the child’s school. An SST facilitator will:
- notify the school counselor to compile information from the student’s cum. file and other sources,
- distribute Teacher Intervention Forms to the student’s teacher,
- put the student on the schedule for the next available SST meeting, and
- Notify all relevant staff about the date of the SST meeting for that student.
