Student Registration

Welcome and thank you for entrusting your child(ren) to the dedicated staff of the Coos Bay School District. Below you will find some of the forms and documentation required upon registering a student for school. Forms can be downloaded, printed and completed prior to registering in person.

Returning Students: *If you were previously enrolled in the Coos Bay School District, please contact the school your child will be entering when you return*

New Students, including Kindergarten: In addition to the Registration Packet, you will need to provide us with the following documents:

Proof of Student's birth date (Example: Birth certificate, a hospital record or a baptismal record)
Immunizations/Health Records
Custody Papers (if applicable)
Academic Records (Copy of recent report card)

ParentVUE Support

To view your current enrollment information, login to ParentVUE.
Click on “Forgot your password? Click here” on the ParentVUE login page if you forgot your password. An email will be sent to the email the district has on file with instructions for how to reset the password. Don't forget to check your junk mail. Contact the school office if you need assistance.

*If you wish to delegate educational duties or rights for your student to another adult, the Power of Attorney for School Purposes form needs to be notarized. The Power of Attorney is valid for 6 months and may be revoked, in writing, at any time.
